You’re right. Before Dad got really bad off, he’d be telling me a story and would occasionally choose a wrong word which changed the story in a major (funny) way! I've seen it firsthand. In that case, the person definitely knew she was going. It wasn't classic Alzheimers. But still, she had many good years, even though she was on the decline. I’ve lived it 3 times in my life, all before 30. One was hard and angry. Another was sad because she wasn’t “their” most of the time. But who she became was a fun and constantly curious person.
