Apply through your regulatory body. There is no easy way but in most cases they are not difficult to get.

What state are you in?
Your best bet would be to visit some local dispensaries and ask around.
The key question to ask when calling around is "Do you have a doctor on site that is able to perform evaluations?" Some licensing facilities do not have doctors or they only operate based off existing medical records. For these places you would need to have a qualifying medical condition on file with your doctor and they would then fax/email your records to the MMJ Certification place. For places with Doctors on site that perform their own evaluations, generally they charge 200-300$ and they will ask you questions about your physical/mental health conditions and then determine whether or not you should (never heard of someone being rejected) receive a MMJ Card.
Do not be intimidated, It is not a scary process! MMJ doctors are there to help and not judge.

This will depend on the state. In Michigan, anyone above the age of about 45 or 50 could easily claim to have one of the ailments considered legally treatable with cannabis. Also, now that we have recreational and a card isn't required, they seem to figure it's unlikely you'd bother getting a card if you didn't really have an ailment that could be treated.

I believe in most states, getting in the MMJ database will nullify your Second Amendment rights. Un-Constitutional? You bet, but to most of these communists, that is just a piece of paper.
