I know this pic sucks but if someone could help with this 3 week old would be great and one looks limp.
I came home and seen this any ideas of what could be causing the issue 2

It would help if you turned off the grow light and use the regular room light so we can see
 
Hard to tell from the image. That media looks REALLY wet to me and it should be damp, but not waterlogged. ??
 
Yes. As it's sitting in the basket. Same as in the pic with different substrate.

Cube should not be in standing water. I soak mine in water and get them fully saturated, then squeeze them out just enough that they don't drip when you hold them without squeezing. (If that makes sense?)

I would get all the water out of there that you can without damaging the sprout.
 
Help me understand please. The seedlings were sitting in clay substrate. Meaning direct water contact and the rock wool is too most?

Rockwool should be damp, not waterlogged. There should be no direct water contact with the rockwool. The waterline should be just below the bottom of the basket.
