Hello im a little confused how I become a caregiver in michigan. Im over 21 and have no drug felonies. I have 2 paitents that gave me copies of all there info to make them my patients but im a little confused on how i do this. i went online and printed lara caregiver change form but i see the first thing they ask for is a caregiver registry id card number I dont have that i never been a caregiver before? do I use the registry id of my patients last caregiver? it says its required so I dont think I can leave it blank.
