sick/dead 6" seedling

hello all
could not find anything online about this.
one of my seedlings got a soggy soft stem about 1/2" up the stem.
it then fell over and died!
4 weeks old , in 4" rockwool ,under 4' t8's, fed nutraflora 50%veg.
6 others all fine
I know its dead and gone, but can anyone tell me what happened?
