I take pics with my phone too an when i hold my phone normal, top to bottom it puts them sideways. Try taking pics while your phone is sideways. Always works for me.

depends on the phone, but don't be too close to the object, and you want to touch the screen to clear the resolution on the point of interest and it should be clear
